I am presently in Debrecen (Hungary) visiting a friend/Medicine 2.0 expert, Bertalan Meskó, aka @Berci, and during my train travels, I came across the following sight:

Miasma or not, feces and bodily fluids go straight from the orifice and are funneled to the train tracks.

For us public health gurus, I thought it would make you say: Got Public Health?
